If you are visiting our office for the first time, your New Patient visit is $95.
Your first visit includes your consultation, evaluation, and first treatment based on the type of care you are seeking.
You are not obligated to pay anything more, purchase a treatment package, or return to our office.
If we believe we can help you, we will explain what Dr. Caldero recommends and review your treatment options and pricing with you. The decision to continue care is always yours.
We also understand that one doctor or one type of treatment cannot help everyone. When you come to our office, we will do our best to understand what is causing your symptoms and determine how we may be able to help you find a solution.

Accidents & Injuries
Medical Lien · MedPay · Third-Party Insurance
If you have been injured in an auto accident or other personal injury accident, we may be able to work with your attorney or the appropriate insurance company so you can receive the treatment you need.
Accident cases are reviewed individually and must be verified before your appointment.
Medical Lien — Attorney Representation
If you are represented by an attorney, we may be able to provide treatment on a Medical Lien, allowing payment for your accident-related treatment to be handled as part of your personal injury case.
MedPay — Medical Payments Coverage
If your automobile insurance policy includes Medical Payments Coverage (MedPay), available benefits may help pay for your accident-related treatment, subject to your policy coverage and limits.
We will bill your insurance company directly and provide the required medical records and documentation for payment directly to our office.
We will verify the necessary information and available MedPay coverage before your appointment.
Third-Party Insurance
If another party was responsible for your accident, we will work with the at-fault party’s insurance company once liability has been accepted and the necessary authorization and documentation are in place.
We will bill the insurance company directly and provide the required medical records and documentation for payment directly to our office.
Before Your Appointment
To help us verify your accident case, please provide the following information:
If you have an attorney: Law Firm Name; Attorney or Case Manager’s Name; Phone Number; Email Address, if available; Claim Number; Date of Accident.
If we are working with an insurance company: Insurance Company Name; Insurance Adjuster’s Name; Phone Number; Email Address, if available; Claim Number; Date of Accident.
An email address is helpful if you have it, but it is not required. A phone number is a good place for us to start.
We will contact the appropriate law office or insurance company to verify the necessary information before your appointment.
Do We Accept Health Insurance?
We are a cash-based practice
Unfortunately, we do not bill regular health insurance directly.
Working with large insurance companies requires significant time and manpower for billing, follow-ups, resubmissions, and collections. We would rather put that time and positive energy into taking care of our patients while offering reasonable, straightforward pricing.
This also allows patients to begin care without having to wait to meet a high deductible before receiving benefits.
PPO Plans
If you have a PPO plan, you may have out-of-network benefits that provide possible reimbursement for some of your treatment.
HMO Plans
HMO plans generally require patients to use in-network providers and typically do not provide out-of-network reimbursement.
We recommend contacting your insurance company directly to verify your individual benefits.
Have a PPO? Ask Us for a Superbill
After you pay for your treatment, we can provide you with a Superbill, which is a paid statement containing the information you may need to submit your treatment to your PPO insurance company.
You submit the Superbill directly to your insurance company for possible out-of-network reimbursement.
The amount of reimbursement, if any, is determined by your insurance company and cannot be guaranteed by our office.

Qualified patients may use 0% financing through Cherry, allowing eligible treatment costs to be divided into manageable monthly payments.
Cherry financing may be used for eligible Pain Relief, Neuropathy, and Pelvic Floor Chair Therapy treatment plans.
